i might be wrong, but i thought otoko was supposed to be ambiguous. like an umbrella term for crossdressing men and pre-op transwomen who all have a cutesy aesthetic. then again i've known people who got into otoko stuff only to realize "whoops i'm actually trans", so maybe i got mixed up there
