Eh, depends on the person, though I get where you're coming from. It shouldn't always be played for crack, because it doesn't fit with the fact that certain characters would be superstitious or come from canons where a doppelgänger is trouble/a very bad thing.

But... honestly, while it's not likely you will meet your doppelgänger, I can state with certainty that somewhere, somewhen, there is or was a person who looked exactly like you. Because there's really only a limited range of human phenotypes. It just seems unlimited because there are approximately forty-six metric shittonnes of genes responsible for appearance.

Case in point: My brother knows a guy who looks exactly like him. As in, even our mother had to pause before figuring out which one was my bro. It is ridiculous how much alike they look, because they are not related. They both have had great fun with this fact, and think it's kind of funny. But then, modern people who are aware of science and aren't particularly superstitious.

A friend of mine has a picture of some dude from like a hundred and something years ago on his facebook (no lie). The guy looks just like him.

For that matter, I am one of those people who get told in all sorts of places that I've never been before (just happened this weekend, in fact), that I look just like someone they've met before. There's a reason that "one of those faces" trope exists.

And I think most games that re-use PBs would just roll with "hey, they kind of look alike but there's no reason to assume they look EXACTLY alike" particularly if they're played differently or using icons from different shows/movies/whatever the PB is from.
